It means that you cut off your stock muffler+tailpipe where the truck bed begins and install a new, usually better-sounding muffler (ex. Flowmaster). On the exit chambers (or chamber) you put small turn-down pipes and the exhaust is 'dumped' under the truck bed.

Really sucks in a dry gravel parking lot. Exhaust pipe won't fit over axle without getting beaten to hell and back due to drop.

Had one on my 99 7.3 PSD, the truck spends most of its time in the woods, it was WAY to loud on the logging roads with the windows down.
